Registration

All SMCISD families—both new and returning—must upload proof of residency, such as a lease agreement, mortgage statement, or electric/water utility bill. Registration cannot be completed until at least one acceptable document verifying residency has been submitted.

Items Required for Registration of a New Student

To enroll a child for the first time, the following is required: 

  • certified copy of the child’s birth certificate if the student is under the age of 11. 
    • Other proof of birth (birth certificate, hospital birth record, school records, etc.) are acceptable if the student is 11 years old or older;
  • current immunization records as required by the Texas Department of State Health Services
  • proof of residence [i.e. deed, lease agreement, OR a recent utility bill (water, electric or gas)];
  • name of prior school attended; copies of prior school records if available
  • Providing a Social Security card or number is optional. SMCISD will not refuse enrollment of any student opting not to provide a social security card/number. In lieu of a social security number, a state identification number will be provided for educational purposes only.
  • photo ID of parent or guardian (Texas law requires that a child must be enrolled by a parent or guardian who has legal control of the student under a court order.)
