Food Allergy Management Plan

  • A parent/guardian must notify the campus school nurse of the student's specific food allergy. After consultation with the school nurse, who will notify the appropriate need-to-know staff members of campus. A Food Allergy & Anaphylaxis Emergency Care Plan must be completed and signed by the parent/guardian and physician. These are to be renewed annually at the beginning of each academic year.
