Employment Verifications & Public Service Loan Forgiveness
All verifications of income, employment or benefit status should email the written request along with borrower's authorization release form to HR@smcisd.net. The appropriate HR staff will complete and submit as indicated.
Employees submitting a Public Service Loan Forgiveness (PSLF) or Teacher Loan Forgiveness applications must complete all areas thoroughly. Incomplete applications will be returned for completion.
Section 1: Borrower Information - Complete all areas prior to submitting
Section 2: Borrower's Signature & Date - A selection must be made. Sign and date.
Upon completion applications will be emailed directly to employee for submission to appropriate entity.
